    The Glendale Healthier Community Coalition (GHCC) is chaired by the Community Foundation of the Verdugos Executive Director and is supported by Glendale’s three hospitals (Glendale Adventist Medical Center, Glendale Memorial Hospital & Health Center, and USC Verdugo Hills Hospital).

    Glendale Adventist Medical Center is an active participant in the healthy city movement through its dynamic involvement in the Glendale Healthier Community Coalition. In operation since 1993, the Glendale Healthier Community Coalition (GHCC), plans and implements projects that promote disease prevention, health education, clean and safe environments, adequate housing, affordable quality education, as well as community revitalization.

    The GHCC mission statement embraces a broad definition of health which includes the wider spectrum of environmental influences impacting the community’s well being. GHCC has brought together over 52 organizations and individual community members. GHCC was originally formed as a collaboration between the City of Glendale and Glendale’s three hospitals: Glendale Adventist Medical Center, Glendale Memorial Hospital & Health Center, and USC Verdugo Hills Hospital. The Glendale Unified School District joined GHCC a year later.

    Recent Projects & Initiatives

    1) Community Care Transitions Project- reduce 30-day readmissions by providing transitional care for discharged patients among the three hospitals. Glendale was identified by Health Services Advisory Group (HSAG) as the first of five demonstration communities working together with HSAG to reduce readmissions;

    2) Developing a population health initiative focusing on diabetes and obesity

    3) Building a local health information exchange (HIE) that enables Glendale’s three hospitals, other providers, and community agencies to share patient information in a HIPAA-compliant environment to better serve the health needs of community residents

    4) Continuing to expand GHCC membership.
